Health Assessments

Nemaha County Community Health Services offers health assessments for children to support school readiness and overall well-being. These assessments are available for school entry physicals as well as Kan-Be-Healthy screenings for children and youth enrolled in KanCare.

Each assessment includes a review of growth and development, vision and hearing screenings, immunization review, and age-appropriate health education. Our goal is to help ensure every child starts school healthy and ready to learn.

Child Wellness Exam

Regular health checkups are an important part of keeping children healthy and thriving. It’s recommended that all children see a healthcare provider at least once each year to monitor their growth, development, and overall well-being.

In Kansas, a health assessment is required before a child can enroll in school or licensed childcare for the first time. At Nemaha County Community Health Services, our skilled and compassionate public health nurses provide comprehensive health assessments that meet all state requirements for school entry.

These assessments include a review of your child’s medical history, a physical exam, immunization record review, and vision and hearing screenings. Our goal is to ensure your child is healthy, up to date on preventive care, and ready to succeed in school.

Kan-Be-Healthy

For families enrolled in KanCare, Nemaha County Community Health Services offers comprehensive Kan-Be-Healthy exams for children and youth ages 1 through 18. These exams provide thorough preventive health screenings conducted by our experienced and professional nursing staff.

The Kan-Be-Healthy exam meets the health requirements for enrollment in Kansas public schools, licensed childcare programs, and Head Start. During the exam, your child will receive a complete physical assessment, including growth and developmental evaluations, vision and hearing screenings, immunization reviews, and health education tailored to their age and needs.
